Your right to know costs up front This page explains how we give you clear pricing information before you receive care, as required by federal law.
JointWorks Physical Therapy, PLLC
Patients' Rights Notice - Your Cost of Care
Under the federal No Surprises Act, you have the right to know your out-of-pocket costs before you receive care. This applies if you are uninsured or have non-government health insurance.
In plain language: Before your visit, we will tell you how much you can expect to pay. If we cannot give you an exact price (because we do not yet know exactly what treatment you will need), we will give you our best estimate.
What we will tell you
When you schedule a non-emergency appointment (or any time you ask), we will let you know whether we accept your health insurance plan.
Your right to a Good Faith Estimate
If we are not in your insurance network, if you choose not to use insurance, or if you are uninsured, you have the right to a written Good Faith Estimate of what you will owe. We will provide this estimate within these time frames:
- Appointment scheduled 10+ business days out: estimate within 3 business days after scheduling.
- Appointment scheduled 3+ business days out: estimate within 1 business day after scheduling.
You can also request a Good Faith Estimate in writing at any time, even if your visit will be covered by non-government insurance. We will provide it within 3 business days. If we cannot give you an exact amount, we will tell you the estimated maximum you would pay.
If your bill is higher than expected
You have the right to dispute a bill if it is $400 or more above the Good Faith Estimate we gave you. For questions or more information about your rights under the No Surprises Act, visit www.cms.gov/nosurprises/consumers or call 1-800-985-3059.
